Form Instructions:
There are 3 forms. When we receive the deposit form with either full payment, or a deposit, we will confirm a student's spot. The other forms must be completed, but may arrive separately or with the deposit form.
All form are enabled for on-line fill in and submission via email. Permission forms require signatures- which may be digital signatures - or the forms may be printed out after typing in the student contact infromation. Priority is given to emailed applications.
PLEASE FILL IN THE FORMS BY TYPING and not printing and handwriting. We reserve the right to reject forms that are hand written. You may email a form by clicking on the YFU logo in the upper corner.
IF YOU SEND THE DEPOSIT WITH A CREDIT CARD BY FAX AND EMAIL AND MAIL. YOU WILL BE CHARGED EACH TIME IT IS SENT. DO NOT SEND MUTLIPLE APPLICATIONS.
All forms require an updated version of adobe acrobat reader - a free program avaialble from Adobe.com. They are up to version 9.0, if you have an old version you will not be able to type into the forms.
